The state's Finance and Administration Cabinet reminds each entity submitting a proposal in response to a personal service contract solicitation of the following:

2. REQUIRED AFFIDAVIT FOR BIDDERS, OFFERORS AND CONTRACTORS CLAIMING RESIDENT BIDDER STATUS (Adobe PDF Format 46KB)

Vendors who respond to Personal Service Contract solicitations must submit a signed affidavit No. 1, above, with their bid or proposal. This affidavit No. 1 is required even though the PSC clause also contains a Campaign Finance section. Vendors who claim a Resident Bidder Status must submit a signed affidavit No. 2, above, with their bid or proposal. These two signed and notarized affidavits must be submitted with your technical proposal.
