The state's Finance and Administration Cabinet reminds each entity submitting a proposal in response to a personal service contract solicitation of the following:
CAMPAIGN FINANCE AFFADAVIT REQUIRED FOR ALL BIDS & PROPOSALS.
- Campaign Finance Law Compliance Form [Updated 4-12-06] (Microsoft Word Format 21KB)
Vendors who respond to Personal Service Contract solicitations must submit a signed Campaign Finance Law Compliance affidavit with their bid or proposal. The Campaign Finance Law Compliance affidavit is required even though the PSC clause also contains a Campaign Finance section. To assure that the vendors comply with this requirement, it is recommended that you paste a copy of the affidavit in your solicitation clauses.
This signed and notarized affidavit must be submitted with your technical proposal.
